Introduction to background check form:
When people hear background check, they immediately think of a company that is requesting background check on its employees or applicants for a new position but that’s not true. In fact there are lots of other areas where a background check is necessary. For instance, you want to rent an apartment and the landlord asks you to fill out a background check form or when you want to purchase a weapon and the shopkeeper asks you to present a background check form. This process is very common when an individual requests for nationality or extension in his residential visa in a country. What is included in a background check form depends on how this form will be used by the person requesting it because each type of form has different set of information and details to be verified and included on the form.
Key elements to include in a background check form:
- Details of employer who request background check:
A background check form starts with putting in the details of the employer who is requesting background check on an individual. This person can be an employer, member of hiring committee or an HR employee whose job is to check backgrounds of newly hired employees. This part should also mention his designation and the company in which he works.
- Details of the applicant whose background is checked:
Next part of the background check should include full details of the applicant or individual whose background is being checked. This individual can be an applicant among dozens of others during a hiring process or he can be an employee in a company and his background is being checked after he is hired. This part should include his full name, contact info, mailing address, social security number and nationality.
- Date on which the request is made:
Usually it is not the case but some organizations have a specific duration for validating background of an individual. For instance if an individual presents background check form that was prepared 2 months ago, the organization has the right to refuse it and ask for a newly filed form. This is why it’s important that the date on which the background check was requested and the date on which it was completed should be mentioned on the form.
- Reason for requesting background check:
Next is the detailed statement from the person requesting the background check mentioning the purpose of this evaluation. There are lots of reasons for requesting background check of an individual. For instance, employers request this check before hiring new employees, banks request this check before issuing loan or credit to an individual, government agencies request this check before issuing passport or nationality to an individual or a university can request this check while an individual submit his research paper for doctorate.
- Consent of the individual for his background check:
There should be a statement included on the background check form that the individual is aware of what this form states, he agrees to reveal the information from his past on the form and he gives his full consent to disclose these details. It’s important for the person requesting the background check to ask the individual to sign right next to this consent statement.
- List of items to be checked:
Then comes the main part of this background check form where the employer or individual requesting the background check includes the items to be checked. This part can be very specific i.e. only checking credit and criminal history of the individual or it can include lots of items i.e. bankruptcy, offenses, traffic violations, arrests, sentences, employment history, travel history and list of previous romantic partners and friends.
- Signature of authorized person:
At the end of the background check form, there is the space for signature. This portion should be signed by the person who checked the background and authenticates that each and every detail mentioned on this form is true to his knowledge.
